Partindo do conto «O alienista » de Machado de Assis, procuro tecer algumas consideraçāes sobre o 'normal' e o 'patológico' com refêrncia a Georges Canguilhem e à sua obra O normal e o patológico: ao procurar compreender o modo como a medicina estabelece o conceito de 'normal' e, em oposiçãāo, o de 'patológico', o seu pensamento foi influenciando um leque alargado de saberes que vāo desde a compreensāo da individualidade e a valonzaçāo da normatividade até ao conjunto das questōes relativas (...) ao trìnómio ética-política-psicologia. Neste artigo, reflicto, a partir da minha prática de patologista, sobre o sentido do diagnóstico morfológico e interrogo-me acerca de como um maior conhecimento das alterações morfológicas se relaciona com uma melhor compreensão das situaçōes patológicas. Sublinho a tensāo entre as dimensōes subjectiva e objectiva ao decidir, na incerteza, como é próprío do processo que leva ao diagnóstico. Defendo ainda a necessidade do raciocínio clínico para a razoabilidade de todo o diagnóstico. Starting from the novel "O alienista" by Machado de Assis, I aim at discussing on the "normal" and the "pathological", referring to Georges Canguilhem and his work The normal and the pathological. Trying to understand how medicine states the concept of "normal" and, as opposed to it, the concept of pathological, Canguilhems thought was influencing a wide range of knowledges, going from the understanding of the individuals and the valorisation of the normativity to the whole of questions concerning the trinomial ethics-politics-psychology. In this paper, I reflect, from my practice as pathologist, on the sense of the morphological diagnosis and I question myself how a dipper knowledge of morphological mutations relates to a better understanding of pathological situations. I highlight the tension between the subjective and objective dimension deciding with uncertainty, as it is peculiar of the process that leads to diagnosis. Also, I support the necessity of the clinical reasoning to the reasonability of every diagnosis. (shrink)

Lower respiratory tract infections are a leading cause of paediatric morbidity and mortality worldwide. Children in low-income countries are disproportionately affected. This is in large part due to limitations in healthcare resources and medical technologies. Mechanical ventilation can be a life-saving therapy for many children with acute respiratory failure. The scarcity of functioning ventilators in low-income countries results in countless preventable deaths. Some hospitals have attempted to adapt to this scarcity by using hand-bag ventilation, as either a bridge to a (...) mechanical ventilator, or until clinical improvement occurs rendering mechanical ventilation no longer necessary. In instances of hand-bag ventilation, an endotracheal tube is first placed. Family members are then asked to play the role of a ventilator, manually compressing a bag repeatedly to inflate the child’s lungs. This approach is fraught with numerous ethical challenges. A careful examination of the data and a nuanced approach to the ethical considerations are imperative. Ethical arguments in support of and in opposition to allowing parental hand-bag ventilation are explored, including the best interests of the child, the child’s right to an open future, beneficence and parental protection, legitimising substandard care, and finally, contextual concerns. An algorithmic, potentially ethically permissible approach to parental participation in manual ventilation is proposed. (shrink)

Although Sign Languages are gestural languages, the fact remains that some linguistic information can also be conveyed by spoken components as mouthing. Mouthing usually tend to reproduce the more relevant phonetic part of the equivalent spoken word matching with the manual sign. Therefore, one crucial issue in sign language is to understand whether mouthing is part of the signs themselves or not, and to which extent it contributes to the construction of signs meaning. Another question is to know whether (...) mouthing patterns constitute a phonological or a semantic cue in the lexical sign entry. This study aimed to investigate the role of mouthing on the processing of lexical signs in French Sign Language (LSF), according the type of bilingualism (intramodal vs. bimodal). For this purpose, a behavioral sign-picture lexical decision experiment was designed. Intramodal signers (native deaf adults) and Bimodal signers (fluent hearing adults) have to decide as fast as possible whether a picture matched with the sign seen just before. Five experimental conditions in which the pair sign-mouthing were congruent or incongruent were created. Our results showed a strong interference effect when the sign-mouthing matching was incongruent, reflected by higher error rates and lengthened reaction times compared with the congruent condition. This finding suggests that both groups of signers use the available lexical information contained in mouthing during accessing the sign meaning. In addition, deaf intramodal signers were strongly interfered than hearing bimodal signers. Taken together, our data indicate that mouthing is a determining factor in LSF lexical access, specifically in deaf signers. (shrink)

_Manual of Regulation-Focused Psychotherapy for Children with Externalizing Behaviors: A Psychodynamic Approach_ offers a new, short term psychotherapeutic approach to working dynamically with children who suffer from irritability, oppositional defiance and disruptiveness. _RFP-C_ enables clinicians to help by addressing and detailing how the child’s externalizing behaviors have meaning which they can convey to the child. Using clinical examples throughout, Hoffman, Rice and Prout demonstrate that in many dysregulated children, _RFP-C_ can: Achieve symptomatic improvement and developmental maturation as a result of (...) gains in the ability to tolerate and metabolize painful emotions, by addressing the crucial underlying emotional component. Diminish the child’s use of aggression as the main coping device by allowing painful emotions to be mastered more effectively. Help to systematically address avoidance mechanisms, talking to the child about how their disruptive behavior helps them avoid painful emotions. Facilitate development of an awareness that painful emotions do not have to be so vigorously warded off, allowing the child to reach this implicit awareness within the relationship with the clinician, which can then be expanded to life situations at home and at school. This handbook is the first to provide a manualized, short-term dynamic approach to the externalizing behaviors of childhood, offering organizing framework and detailed descriptions of the processes involved in _RFP-C_. Supplying clinicians with a systematic individual psychotherapy as an alternative or complement to PMT, CBT and psychotropic medication, it also shifts focus away from simply helping parents manage their children’s misbehaviors. Significantly, the approach shows that clinical work with these children is compatible with understanding the children’s brain functioning, and posits that contemporary affect-oriented conceptualizations of defense mechanisms are theoretically similar to the neuroscience construct of implicit emotion regulation, promoting an interface between psychodynamics and contemporary academic psychiatry and psychology. Manual of Regulation-Focused Psychotherapy for Children with Externalizing Behaviors: A Psychodynamic Approach is a comprehensive tool capable of application at all levels of professional training, offering a new approach for psychoanalysts, child and adolescent counselors, psychotherapists and mental health clinicians in fields including social work, psychology and psychiatry. Procedure manuals and textually mediated deathThe procedure manual as a document represents the practice of nursing care. Analysis of such manuals allows us to explore discourses of nursing and the ways in which they frame nursing practice. A critical analysis of a hospital procedure manual using discourse analysis was undertaken. A specific excerpt concerning ‘Last offices’ is used as an example of the institutionalisation of organisational values and beliefs as these influence nursing care. ‘Last offices’ directs nursing practices (...) related to the death of a patient, and the laying out as preparation for the funeral director or coroner. The analysis shows that procedure manuals reproduce hegemonic ideologies of nursing. The analysis also shows how nursing practices are constituted by intersecting forces of differing locations such as the hospital as an organisation, the healthcare system and the wider society within which these function. El manual escolar sigue siendo un recurso didáctico clave en la enseñanza de la historia. En Educación Primaria y Secundaria pervive su uso, a pesar de las numerosas reformas educativas llevadas a cabo desde hace siglo y medio. Este trabajo pretende analizar su vigencia, estructura y cambios a lo largo del tiempo. Los cada vez más numerosos estudios se han escrito desde una perspectiva española o portuguesa, así que interesa conocer la evolución paralela y establecer comparaciones. Se analizan las (...) escuelas historiográficas y pedagógicas que han influido en la evolución de los currículos educativos. Pointing by apes is near-ubiquitous in captivity, yet rare in their natural habitats. This has implications for understanding both the ontogeny and heritability of pointing, conceived as a behavioral phenotype. The data suggest that the cognitive capacity for manual deixis was possessed by the last common ancestor of humans and the great apes. In this review, nonverbal reference is distinguished from symbolic reference. An operational definition of intentional communication is delineated, citing published or forthcoming examples for each of the (...) defining criteria from studies of manual gestures in apes. Claims that chimpanzees do not point amongst themselves or do not gesture declaratively are refuted with published examples. Links between pointing and cognitive milestones in other domains relating means to ends are discussed. Finally, an evolutionary scenario of pointing as an adaptation to changes in hominid development is briefly sketched. (shrink)

This manual is the product of a normative research program carried out over four years with Spanish-speaking populations in two geographically distinct regions: Madrid, Spain and the USA/Mexico border region. The manual describes a comprehensive system of procedures and normative data designed to assist the clinical researcher and the clinical practitioner in the neuropsychological assessment and diagnosis of adults whose main language is Spanish. Together the procedures comprise a brief and practical battery of eight tests for a basic (...) examination of attention, learning and memory, and executive function. The level of performance on these tests is assessed according to normative data. This is accomplished by adjusting test scores according to demographic criteria that simultaneously take into account the traditional factors of age and education, as well as the patient's place of origin. Intensional logic is the technical study of such intensional phenomena in human reasoning as modality, knowledge, or flow of time. These all require a richer semantic picture than standard truth values in one static environment. Such a picture is provided by so-called possible worlds semantics, a paradigm which is surveyed in this book, both as to its external sources of motivation and as to the internal dynamics of the resulting program. In recent years, tech evangelists have made headlines predicting that in the future manual driving will be outlawed. This essay will investigate the question whether a ban of human driven cars can be defended on moral grounds in a future scenario in which autonomous cars are going to be significantly safer than manually driven cars. This article will argue that in such a future scenario manually driven cars, for moral reasons, indeed should be banned from participating in regular traffic. (...) Since the moral argument for outlawing manually driven cars will likely be met by resistance by car-aficionados, in the final part of the paper, we are devising a proposal for reconciling the strong moral case for a ban of manually driven cars with the widespread fondness of manual driving. This manual presents a carefully researched, detailed psychodynamic treatment program for the alleviation of a transdiagnostic range of primary Axis I anxiety disorders, including panic disorder, generalized anxiety disorder, social anxiety disorder, post-traumatic stress disorder, and related psychological problems. First exploring the principles of psychodynamic theory and formulation, the authors then present a three-phased process of Panic Focused Psychodynamic Psychotherapy-Extended Range : initial evaluation, interpretation of central conflicts and defense mechanisms, and termination. Recording of neural activity during grasping actions in macaques showed that grasp-related sensorimotor transformations are accomplished in a circuit constituted by the anterior part of the intraparietal sulcus (AIP), the ventral (F5) and the dorsal (F2) region of the premotor area. In humans, neuroimaging studies have revealed the existence of a similar circuit, involving the putative homolog of macaque areas AIP, F5, and F2. These studies have mainly considered grasping movements performed with the right dominant hand and only a few (...) studies have measured brain activity associated with a movement performed with the left non-dominant hand. As a consequence of this gap, how the brain controls for grasping movement performed with the dominant and the non-dominant hand still represents an open question. A functional magnetic resonance imaging (fMRI) experiment has been conducted, and effective connectivity (dynamic causal modeling, DCM) was used to assess how connectivity among grasping-related areas is modulated by hand (i.e., left and right) during the execution of grasping movements toward a small object requiring precision grasping. Results underlined boosted inter-hemispheric couplings between dorsal premotor cortices during the execution of movements performed with the left rather than the right dominant hand. More specifically, they suggest that the dorsal premotor cortices may play a fundamental role in monitoring the configuration of fingers when grasping movements are performed by either the right and the left hand. This role becomes particularly evident when the hand less-skilled (i.e., the left hand) to perform such action is utilized. The results are discussed in light of recent theories put forward to explain how parieto-frontal connectivity is modulated by the execution of prehensile movements. (shrink)

Vilfredo Pareto's Manual of Political Economy is a 'classic' study in the history of economic thought. It is not only one of the leading works in the Lausanne tradition of economics, which centres on the theory of general equilibrium, it is one of the most important books in the history of neoclassical economics. This 'critical edition' of Pareto's Manual of Political Economy is a very significant work for two main reasons. First, it is the only variorum translation of (...) the 1906 Italian language Manuale di Economia Politica and the 1909 French language Manuel d'Économie Politique. Second, it includes extensive contributions by the editors, all of whom are authorities on the work of Pareto. It total, the editorial contribution are around two hundred pages and range from brief annotations, to extended editors' notes and detailed notes on the 1909 mathematical appendix. (shrink)

In this essay on man Mr. Fuller expresses what may well be his penultimate view of the human condition. Here, in a mood at once philosophical and involved, Mr. Fuller traces man’s intellectual evolution and weighs his capability for survival on this magnificent craft, this Spaceship Earth, this superbly designed sphere of almost negligible dimension in the great vastness of space. Mr. Fuller is optimistic that man will survive and, through research and development and increased industrialization, generate wealth so rapidly (...) that he can do very great things. But, he notes, there must be an enormous educational task successfully accomplished right now to convert man’s ten­dency toward oblivion into a realization of his potential, to a universe-exploring advantage from this Spaceship Earth. It has been noted that Mr. Fuller spins ideas in clusters, and clusters of his ideas generate still other clusters. The concept “spaceship earth” is Mr. Fuller’s, and though used by Barbara Ward as the title of a work of her own the idea was acknowledged by her there as deriving from Mr. Fuller. The brilliant syntheses of some fundamental Fuller principles given here makes of this book a microcosm of the Fuller system. (shrink)

Este examen nos permitirá entender la manera como se articula dicha tesis con otros sectores de su filosofía. En particular, argumento que la tesis de la indeterminación de la traducción, estrictamente hablando, cumple· una función legitimadora en el sistema quineano. Following the Second Vatican Council, the predominant trend in Catholic moral theology has been decidedly antagonistic toward the tradition that dominated moral theology before the Council, namely the use and formulation of ecclesiastically-approved “manuals” or “handbooks” of moral theology, the contents of which chiefly involved general precepts of morally good and bad behavior as well as the extension of those precepts to particular cases. In this paper, I will oppose the dominant anti-manual trend. More particularly, I will first sketch (...) what I take to be the central aspects of the manual tradition. Second, I will provide several arguments in favor of this tradition. Last, I will raise and respond to objections to this tradition that feature prominently in the works of Pinckaers and Cessario.
